Student of Concern Form
This form is intended for sharing non-emergency concerns about a student’s well-being and/or behavior. If you believe someone is in immediate danger, experiencing extreme distress, or at risk of harming themselves or others — or if you feel unsafe — please call 911 right away and then complete the form.
Information shared through this form is reviewed by the Office of Student Advocacy and Support at University of Nebraska–Lincoln. Our team assesses the situation and coordinates supportive outreach based on the student’s needs. Please share your name and contact information so our team can follow up if we need additional context to best support the student. You may request that your identity remain anonymous to the student at the end of the form.
Referral Form
This referral form should be used if a student has indicated they would like supportive, proactive outreach from SAS. If you are concerned about a student's safety or behavior, please use the Student of Concern form.
Please use our referral form below to refer a student who has shared a personal, academic, or life concern and is open to outreach from Student Advocacy & Support (SAS). The information you provide will be reviewed by the Office of Student Advocacy and Support and appropriate outreach will take place.
If you have already encouraged a student to connect with us you can still fill this form out so that we have additional contextual information and can follow up with you if needed.
